Specimen

2 hour timed urine collection after an overnight fast.

The patient should not consume meat or gelatine-containing products (eg, ice cream) for the 24 hours prior to the collection.

Method

Spectrophotometry. Results are expressed as a ratio to creatinine.

Reference Interval

Dependent on age and lean body mass. Adult: <35 mmol/mol creatinine.

Application

Assessment of bone resorption eg, in the monitoring of Paget’s disease and osteoporosis.

Has largely been replaced by other markers of bone turnover.

Interpretation

Hydroxyproline output indicates the level of active bone turnover and associated collagen catabolism.
